We have an exciting opportunity within our care navigator service across Teesside, where you will join our friendly and inspiring team as a Mental Health Care Navigator. Your role will be a permanent position working full time, 37.5 hours per week based in the service. You will work Monday to Friday 08:00am -16:00pm, however some flexibility is required depending on release arrangements and external meetings with partner agencies.

Our mental health care navigator service provides the service in the 7 North East Prisons in partnership with Tees, Esk and Wear Valley NHS Foundation trust and other Rethink colleagues. The aim of the service is ensure our patients are linked to appropriate services and support networks prior to release; and have post release access to health and social care services in parity with the wider community; reducing the risk of social exclusion, mental health relapse or potential re-offending.

What will you be doing?

Supporting individuals with issues pertinent release such as housing, debt, benefits, healthcare, family links, general wellbeing support and employment.
Be involved in the peer mentoring scheme and service user forums.
Working with offenders to identify long-term resettlement plans so that independence in the community is sustained and the likelihood of reoffending is reduced.
Taking on a caseload of work with offenders and will work collaboratively with colleagues and external partners/agencies to achieve the best possible outcomes for service users.
Working in a pro-active, solution focused way, building effective working relationships with other agencies and providers to ensure the best possible patient care

Do you have what we are looking for?

Experience of working within professional boundaries in a challenging environment
Experience in a comparable role, preferably in housing, social services, residential care or supported housing.
In depth experience of advice work or related housing and resettlement work
The ability to make a comprehensive assessment of an individual’s situation and needs; to develop, implement and review risk management plans and devise support plans to meet these needs.
Ability to use supervision and personal development positively and be self-reflective in own personal and professional development
Ability to function effectively within a prison environment, to work under pressure and manage stress

You will be required to travel as part of the role which can include travelling regionally. We therefore need you to have a full UK driving licence with access to transport.
