Applicants must be RMN qualified, hold a current Non-Medical Prescribing qualification, have experience of working within a Memory Assessment Service and have in depth knowledge of Dementia and contemporary interventions.
This is an exciting opportunity for a suitably qualified and experienced professional to clinically lead the developing East Riding Memory Service. The service is currently under transformation to become a Specialist Service offering memory assessment, diagnosis and post diagnostic interventions to those residing in the East Riding of Yorkshire.
The Team will partner with third sector colleagues and other agencies across the community to ensure the current and progressing needs of those with a diagnosis of Dementia are planned and supported. The Clinical Lead will aspire to being part of the provision of a truly integrated service.
The Team will deliver local services within venues across the East Riding, enhancing professional relationships with colleagues in the 7 Primary Care Networks. The post holder will have an understanding of the Frailty Pathway for East Riding residents and how this links with the Dementia Pathway.
The NMP Clinical Lead will be responsible for the Clinical Leadership of the team, providing clinical supervision to those within the team, they will be supported operationally by the Team Leader and Service Manager to ensure all Key Performance Indicators are met.
The successful candidate will provide assessment, formulation, diagnosis and feedback for patients. With memory impairment/mental health problems and have skills in communicating with these patient groups and their care givers. They will also, where appropriate, prescribe medication within prescribing guidelines.
The post holder will receive and give regular supervision and attend relevant training identified through Knowledge and Skills Framework processes and their individual performance and development review (PADR).
