We work from a variety of community settings, such as three community hospitals, community clinics, schools, nursing homes and primary care settings, as well as within peoples’ homes to provide over 40 services to children, families and adults.

We provide services across Essex and in Cambridgeshire (including Peterborough), Suffolk and Norfolk as well as two London boroughs, Waltham Forest and Redbridge. We have an income of approximately £67 million and employ over 1,050 people and serve communities with a total population of more than 1.9 million people.

What we arelooking for:

  • To support the PCN with pro-actively assess the needs of their population and identify patients who would benefit from social prescribing support - E.g. carers, people recently bereaved, people with unhealthy lifestyle choices and those suffering with loneliness and isolation.
  • To provide personalised, practical support to patients, their families and carers enabling them to make decisions that will improve their health and well-being outcomes and to increase their independent living and support by connecting them to community initiatives for sustainability.
  • Take a holistic approach, based on the person’s priorities and the wider determinants of health using coaching techniques and a develop trusting relationships by giving people time and focus on ‘what matters to me’ and co-produce a personalised care and support plan.
  • The role will require managing and prioritising your own caseload in accordance with the needs, priorities and support required by individuals on the caseload. It is therefore vital that the Social Prescriber has a strong awareness and understanding of when it is appropriate or necessary to refer people on to other health professionals/agencies, when the person’s needs are beyond the scope of the role – e.g. when there is a mental health need requiring a qualified practitioner.
  • Work collaboratively with other members of the multi-disciplinary team, colleagues, local partners and community groups to contribute towards supporting local resources for sustainability and to help ensure that community assets are nurtured.
  • Recognise and remedy gaps in provision by sharing intelligence, regarding shortfalls or problems in local provision, with commissioners and local authorities.

Provide’s Essex Lifestyle Service aims to empower the people of Essex to make healthy lifestyle choices, and live healthier lives. The service works with clients to discuss needs and then offer up to date and evidence-based help and support, to enable long-term positive changes to lifestyles. We offer Stop Smoking support, guidance on diet and exercise and help to improve self-esteem. We also offer classes to support people suffering from Long Term Conditions in Mid Essex. In addition to the services we provide, we are able to signpost people to appropriate support provided by partner organisations.
