An excellent opportunity has been made available by an established housing provider in Bristol for a Support Coordinator - Recovery Navigator.

This is a Temp to Perm opportunity on a full time basis (35 hours) per week.

Candidate must be flexible to work across the week on a shift rota basis.Early shifts are 08:00 am - 14:00 pm and late shifts are 14:00 pm - 20:00 pm.

The rate of pay is £10.10 PAYE (Plus Holiday Pay) or £13.00 per hour via Umbrella.

Candidate must have a Full UK Drivers License and access to a vehicle.

As a Support Coordinator, you will be required to build trusting and collaborative relationship with the service users.

You will need to carry out role adhering to Mental Health Bristol Partnership's policies, procedures, values, code of conduct and current legislation using a Recovery orientated, psychologically informed, equalities based approach.

You will need to Co produce, implement and review recovery plan, crisis plan and other self-management tools e.g. Wellness Recovery Action Plan, in partnership with service users in line with best practice.

Part of your role will be to case manage and co-ordinate support, referring and liaising effectively with all professionals, agencies and other parties involved in service user's support in order to deliver the support package.

You will need to work closely with colleagues with the service and build strong partnership working with other agencies such as Health and Social Care, Voluntary and Community Service. Support/ facilitate service users to access services.

As a Support Coordinator, you will need to identify, assess, contribute, maintain, feedback and manage risk to ensure risk is minimised.

You will be required to work in a safe manner using a positive risk-taking approach, ensuring the health and safety policies of Mental Health Bristol are implemented and fully adhered to ensure all safeguarding issues are properly managed.

You will need to have up-to-date safeguarding training and ensure that Safeguarding policies and procedures are fully adhered to and that a Think Family approach is embedded within working practice.

Part of your role will be to enable delivery of the recovery plan and to enable service users to make informed decisions about their mental well-being, maximising their independence and providing advocacy role where appropriate.

Build trusting and collaborative relationship with the service user.

Furthermore, you will be tasked with working in a culturally sensitive way for all service users, tailoring service to meet their individual needs, challenging stigma and discrimination, advocating for person if appropriate, and ensuring up-to-date community knowledge.

Part of your role will be to work with a range of needs e.g from young people to older people, LGBT service users, disabled people and with people with a range of backgrounds and ethnicity.
