Job Description :

  • You will be the Patient Pathway Navigator for the new Wessex Rapid Diagnostic Service and will have an instrumental role in the running of this new service.
  • You will be able to work autonomously advocating on behalf of the patient through timely liaison and co-ordination with colleagues in primary and secondary care to ensure appropriate navigation.
  • You will be motivated and enthusiastic with commitment to delivering a quality service to our patients.
  • You will be joining an exciting, new and developing service, the Wessex Rapid Diagnostic Service (RDS).
  • This is a regional service which is delivered using a virtual model, to provide a cancer fast track referral route for patients seen in Primary Care with non-specific symptoms.
  • The RDS aims to speed up diagnosis for this typically hard to diagnose group and to improve overall patient experience.
  • On a day to day basis you will be speaking to patients and supporting them with navigation through their pathway.
  • You will be ensuring timely booking and reporting of investigations, management and escalation of delays and issues in line with the optimal timed pathway.
  • You will be providing Oversight and tracking of the process of diagnosis whilst ensuring the collection, verification and input of complete and accurate data for the purpose of meeting Cancer Outcomes & Services Dataset (COSD) requirements.
  • You will have detailed organisational knowledge of health service policy in relation to cancer targets/COSD and have experience of working with and awareness of NHS Waiting times standards.
  • You will have experience of implementing service improvement projects.
  • You will have experience of working in a pathway coordination/navigation role or similar. WHAT WE CAN OFFER YOU
