Hours: 37 per week
Contract: 12 month fixed term (with the possibility of extension)
Salary: £24,487 - £26,759 per annum depending on experience
Role Location: Central Bristol & Fishponds
We have an exciting opportunity for an experienced Senior Navigator as part of the new and innovative Specialist Community Forensic Service (SCFT). Working within this pilot service (with the possibility of extension) you will work in partnership with the AWP to support early discharge into the community for Forensic patients.
About the role:
The focus of the role is to work alongside AWP to provide appropriate and effective interventions to people with long term mental health needs on their discharge from hospital You will enable and assist them to meet daily health, social care and well-being needs in line with personal recovery goals, and facilitating engagement with mainstream services.
You will have a case load and will act as care coordinator for a number of service users. This will include both initial and ongoing assessments of needs, goals and aspirations, the development and co-production of plans, and provision of support for the service user... The role of Senior Recovery Navigator will also involve carrying out specific health or social care assessments and interventions whilst supervising and support a Team of Recovery Navigators. You will also manage a team of two Recovery Navigators.
You will have:
Proven knowledge of the support needs of people with significant mental health difficulties, criminal justice backgrounds and complex needs.
proven knowledge of risk management and safeguarding policies and procedures relating to adults
Experience of supervising others
Whilst this role is based in Central Bristol and Fishponds, you will be expected to travel with Bristol, North Somerset, South Gloucestershire to meet service users/clients. As such you should have access to your own transport.
